*All applications must be received by June 14^th, 2019

*One application per household.

*Duplicate applications will be disqualified.

*Incomplete applications will be denied.

*Winners will be selected by Lottery.

*Applications will only be accepted

They are as follows:

Income Documents Required at time of Application Submission

1. If you are SELF EMPLOYED:

• Federal Tax Returns including      ALL schedules for the prior year (2018)

• W2’s and all 1099’s for the prior year      (2018)

• 3 Months of ALL bank statements – Must      include ALL pages of bank statements, single page statements only.

2. If you receive CHILD/SPOUSAL      SUPPORT:

• 3 Months of ALL Bank Statements - Must      include ALL pages of bank statements, single page statements only.

• Federal Tax Returns including      ALL schedules for the prior year (2018)

• W2’s for the prior year (2018)

3. If you receive a PAYCHECK      FROM AN EMPLOYER:

• 8 Most recent, consecutive pay stubs

• 3 Months of ALL Bank Statements - Must      include ALL pages of bank statements, single page statements only.

• Federal Tax Returns including      ALL schedules for the prior year (2018)

• W2’s and all 1099’s prior year (2018)

4. If you receive income from SSI,      PENSION, UNEMPLOYMENT or FAMILY SUPPORT:

• SSI Award Letter- If applicable      (must be dated within 30 days)

• Pension/Annuity Statement- If      applicable (must be dated within 30 days)

• Unemployment Benefits letter – If      applicable (must be dated within 30 days)

• 3 Months of ALL Bank Statements - Must      include ALL pages of bank statements, single page statements only.

• Federal Tax Returns including      ALL schedules for the prior year (2018)

• W2’s and all 1099’s prior year (2018)

5. If you are a STUDENT:  

Full-time student households must meet one of 5 IRS exceptions:

·       Any member(s) of the households is married and is filing (or is entitled to file) a joint tax return

·       Be receiving assistance under Title IV of the Social Security Act (AFDC, Cal Works, or TANF not SSA/SSI)

·       Have recently existed the Foster Care System

·       Be enrolled in a job training program under the JTPA or WIA or under another similar Federal, State or local law

·       Be a single parent with a child (or children) and neither the parent or children are claimed as a dependent on a third parties tax return
